Disability Assessment and Treatment Letter for an Emotional Support Animal (ESA):
Even if your pet is already registered as an Emotional Support Animal with an official registration agency, you will still need a letter prescribing your need for an emotional support animal written by a psychologist or other licensed Qualified Mental Health Professional (QMHP).

The Treatment / ESA Recommendation Letter:
We understand that most clients may experience some degree of emotional distress and seek services from us assuming they meet the criteria. Please understand that not everyone will qualify.
Ethically, Tina C. Christian, LPC, NCC has to professionally feel that a client clinically qualifies before diagnosing a client as “disabled”. After the comprehensive intake evaluation assessment, and engaging and assessing the client during the telephone and online counseling sessions, Tina C. Christian, LPC, NCC will designate the appropriate diagnosis and determine if the client is impaired by the negative symptoms associated with the diagnosis. An Authorization for Use of Protected Health Information form “MAY” be required, if felt necessary by the therapist to complete the assessment.
Only “if” criteria is met, will a treatment/ESA recommendation letter be written, signed, and sent.

The assessment and one ESA letter to either an airline, landlord or university for just $225 (assessment fee is $200 and the ESA letter is $25).

The Intake and Diagnostic Evaluation Assessment Process:
This process begins when a client intake is completed online. This intake includes a comprehensive evaluation to collect background information, assess psychological functioning, and a complete medical and mental health history.
Authorization for Use of Protected Health Information form “MAY” be required, if felt necessary by the therapist to complete the assessment.
The Intake and Diagnostic Evaluation Assessment is done by the use of 256-bit encryption to protect the privacy of Personally Identifiable Information {PII) and Protected Health Information (PHI). Your information will be protected consistent with the principles of the Privacy Act of 1974, and the Health Insurance Portability and Accountability Act of 1996 (HIPAA), HIPAA Privacy Rule, Security Rule, Enforcement Rule, and final Omnibus Rule.
The privacy policy explains our information practices when you provide PII or PHI to us, whether collected online or offline, or when you visit us online to browse, obtain information, or conduct a transaction. PII or PHI may include: your name, email, mailing and/or home address, phone numbers, Physical or mental health diagnosis and information, or other information that identifies you personally.
Once this comprehensive Intake and Diagnostic Evaluation Assessment has been analyzed, a Tina C. Christian, LPC, NCC reviews the results, focusing on the client’s subjective responses and medical/mental health history.
Using the comprehensive evaluation results and other ancillary information (e.g., information from other health care providers, etc.) Tina C. Christian, LPC, NCC will contact the client to set up a telephone consultation and online counseling session(s) to further evaluate the client’s mental and emotional status.
These counseling sessions are required to formulate an informed disability determination and cannot be avoided.
A valid email address and telephone number is “required” for the licensed clinician to contact you for the necessary counseling sessions. Telephone/Online counseling sessions are included at no additional charge. A Treatment/ESA Recommendation Letter is Not Guaranteed! Although this service includes both the disability assessment AND a treatment recommendation letter for an Emotional Support Animal (ESA), purchase of these services does NOT guarantee a client will qualify as disabled. The assessment and treatment recommendation letter for an ESA is valid for 12 months, at which time a new assessment and treatment recommendation letter are required. The online assessment uses 128-bit encryption to keep your responses private. The findings of our assessment, along with the DSM-5 diagnosis are included in the formal clinical report we prepare and send to you.
What is a verification form?
It has become common practice for airline companies, property managers, and colleges/universities to require a person’s therapist to complete and return a third party verification form. This is often in addition to the treatment recommendation letter the client may have.
UNITED AIRLINES NOW REQUIRES THIS FORM TO BE COMPLETED FOR ALL PASSENGERS FLYING WITH AN ESA.
A verification form is an additional service that may or may not be required. This will require Tina C. Christian, LPC, NCC to complete and send additional paperwork to a landlord and/or airline. Therefore, there is an additional $25 charge per form requested.
The form can be from 1 – 4 pages in length, and is often faxed or emailed to the therapist by the airline, property manager, or college (although the form may also be submitted by the client). This third party verification calls for a variety of very specific and personal details about the client, details of his/her emotional/psychological disability, length of treatment, acceptable treatment modalities, alternative treatment other than an ESA, the client’s prognosis, and much more.
The Verification Form Completion Service may be ordered directly from this page (at a reduced cost) or ordered at a later date. When the Verification Form Completion Service is ordered, your electronic record is noted, and when we receive a verification form, we automatically complete, sign, and return to you, the airline company, property manager, or college, as appropriate. We only contact you if we have no signed and proper information release form in your record.
Due to the nature of the therapeutic relationship Tina C. Christian, LPC, NCC has with clients, it cannot be guarantee the access to all the information requested in the form, in which case “unknown” or a similar response will be entered, or you may be contacted as appropriate. Although every attempt to establish and confirm it necessary for you to have an emotional support animal because of your mental health disability, the final determination rests with the airline, property manager, college, or university.
Process Time-frame
Generally, the process can take a little as couple days to a couple weeks.
The online clinical intake and diagnostic evaluation assessment process should generally take the client approximately 30-45 minutes (possibly more) depending on required sections of the intake and provided that all requested information is available to you while completing this form (psychological and medical history). Once completed, it is analyzed by Tina C. Christian, LPC, NCC within 24 hours. The client will be emailed to schedule dates and times for the telephone consultation and online counseling session(s). This begins the actual assessment and diagnostic process.
How long the process takes will be determined by the availability of the client, the number of counseling sessions necessary for Tina C. Christian, LPC, NCC to make a legitimate and well-founded evaluation.
Authorization for Use of Protected Health Information form “MAY” be required, if felt necessary by the therapist to complete the assessment, which may also determine the time-frame necessary to complete the process.
Generally, the process can take a couple days to a couple weeks “if” criteria is met, will a treatment recommendation letter be written, signed, and sent to the client.
If during the evaluation Tina C. Christian, LPC, NCC recommends additional counseling , the client will be informed and can schedule follow-up sessions with the therapist. If, recommended it is important to either follow up with Tina C. Christian, LPC, NCC or another licensed professional mental health provider.
IMPORTANT NOTES:
1) Test scores, raw data, test questions/answers, and subjective results of interview questions, and counseling session notes are not available to clients or other inquirers. Only the specific services purchased will be provided.
2) Tina C. Christian cannot guarantee a property manager or airline company will not inadvertently or intentionally violate law by discriminating against you, despite possessing a valid treatment recommendation letter.
3) You are paying to be assessed using an online client intake diagnostic and evaluation assessment, and counseling sessions with Tina C. Christian, LPC, NCC. Once you have completed the assessment with Tina C. Christian, LPC, NCC, a FULL refund for services will not be warranted, if a client is not qualified as disabled.
Assessment is “required” to determine eligibility. Therefore, refunds are limited. Once you have completed the assessment, had the phone consultation and online counseling session(s) only additional services requested outside of the standard $200 assessment fee will be refundable.
